I don't have a template, but have done this a few times. You could reach out to one firm and ask them what they need to give you a good estimate. You can use this information to go to a few more firms. I would think a local firm or maybe a mid-tier firm (Grant Thornton, McGladrey) would be able to do this. You may want to give them information that will make their audit simpler such as the fact that your company can do a lot of the pre-work or prepare as much as possible for the auditors. Also, which firm has more healthcare clients in your area? That would be the firm I would target as they should be more efficient and more knowledgeable on your sector
