In my opinion business lost is primarily attributed to "follow" - follow-up and follow-through. It all comes down to permutations of the same word. Customers / Clients transact business when they have a need. It is critical in Sales to be the solution when the need arises. The perfect timing is hard to capture, so follow-up is important. So now that you have the customer / client it is important to provide the product or service you agreed to provide. You need to follow-through.
To add to Regis comment, having knowledgeable front line service staff that have sharp listening skills, understand products / service offerings, and combine these skills to create real opportunities that close.
